Mandibulofacial dysostosis with microcephaly, Guion-Almeida type (MFDGA) is a rare multiple congenital anomalies syndrome characterized by malar and mandibular hypoplasia, microcephaly, ear malformations with associated conductive hearing loss, esophageal atresia, cleft palate and distinctive facial dysmorphism. A 61-year-old Japanese woman was admitted to our hospital for further examination of generalized bone pain. At age 46 years, the patient was diagnosed with Fanconi syndrome associated with benign monoclonal gammopathy of undetermined significance. At age 56 years, alkaline phosphatase (ALP) increased to 448 IU/mL. A bone biopsy was performed, and osteomalacia was diagnosed.
